(A) Documents and data.
(1) The documents and data listed in § 154.045 shall be a required part of the application for approval of a preliminary plat under this chapter.
(2) An application for approval of a preliminary plat shall include the following information of that listed in this section shall be included in the preliminary plat as required by division (A)(3) below:
(a) Boundary lines, bearings, and distances: The error of closure shall be of third order survey and no discrepancy between computed and measured distances shall exceed 1 part in 5,000 parts;
(b) Easements: Location, width, and purpose;
(c) Streets on and within 1,000 feet of the tract: Name, right-of-way width and location; type, width, and elevation of surfacing; legally established centerline elevations; sidewalks, curbs, gutters, culverts, and the like;
(d) Utilities on and immediately adjacent to the tract: Location, size, and invert elevation of sanitary, storm, and combined sewers; location and size of water mains; location of gas lines, fire hydrants, electric and telephone poles, and street lights; if water mains and sewers are not on or adjacent to the tract, indicate the direction and distance to, and size of nearest ones, showing invert elevation of sewers. The City Engineer may waive any part or all of this required information; provided that, he or she does so in writing specifically stating that which is waived. If a waiver is granted, it shall be included with the application at the time the application is filed;
(e) Ground elevations on the tract, based on U.S.G.S. datum; for land that slopes less than approximately 2%, show spot elevations at all breaks in grade along all drainage channels or swales, and at selected points not more than 100 feet apart in all directions; for land that slopes more than approximately 2%, show either contours with an interval of not more than 5 feet if ground slope is regular and the information is adequate for planning purposes, or contours with an interval of not more than 2 feet, if necessary because of irregular terrain or need for more detailed data for preparing plans and construction drawings;
(f) Subsurface conditions on the tract; location and results of soil percolation tests, if individual disposal systems are proposed or necessary. The tests shall be as required by the County Health Officer;
(g) For those subdivisions outside the corporate limits of the city where plans are to develop their own water supply, a detailed hydrology report shall be submitted as to quantity, quality, and effects on availability of water to adjacent water users. If the plat provides for construction of 1 or more central water supply systems, the plan or plat shall assure the availability of sufficient potable water, and to that end shall describe the source, location, quantity, and quality of the water:
1. If the supply is to be from surface water sources, the plat must contain a report by a qualified person on the hydrology of the supply, including drainage area; historical run-off records or estimates; drought frequency analysis and prediction; and description of the storage, treatment, and distribution facilities;
2. If the supply is to be from groundwater sources, the plat must contain a report, by a person qualified in groundwater hydrology, presenting a detailed geologic map of the lands to be developed; a reconnaissance geologic map of adjacent lands showing the hydrologic boundaries on the aquifer system from which water will be taken; information on depth to, water in, and extent and thickness of the aquifers; probable yields of wells based upon pumping tests; chemical quality of water; and logs and yields of wells; maps showing contours of the water levels in wells and thickness of saturation of the principal aquifers; information on present or potential sources of groundwater pollution and computations of the effects of the proposed water withdrawal on water levels and natural discharge within and exterior to the proposed development; and/or
3. If the water supply is to be provided from surface supplies or from groundwater supplies within declared underground basins, the water supply plan accompanying the final plat or final site plan must include a permit issued by the State Engineer for the use of the quantity of water required.
(h) Other conditions on the tract: Water courses, arroyos, rock outcrops, wooded areas, and other features;
(i) Other conditions on land within 1,000 feet of the tract: Owners of record of unplatted land; for platted land of record refer to subdivision plat by name, recordation date, and indicate developed lots and type of development. The City Manager may modify the distance for which this data is required; provided that the modification is in writing stating his or her reasons therefor and is included with the application;
(j) Zoning on and within 1,000 feet of the tract. This requirement may be modified or waived by the City Manager, if it is in writing and is included with the application;
(k) Title and certificates: Present tract designation according to official records in the County Clerk’s office; title under which the proposed subdivision is to be recorded, with name and address of owner; notation stating acreage, scale, true and magnetic north arrows, U.S.G.S. datum and benchmarks, if any; certification of the engineer or land surveyor licensed in accordance with the laws of the state who prepared the plat, which certification is mandatory; and a metes and bounds description of the tract; and/or
(l) Proposed public improvements: Public facilities, thoroughfares, or other public improvements or lands shown on the adopted master plan for future acquisition or development on and near the tract.
(3) As part of the application for preliminary plat approval, the subdivider shall submit 6 blueline or blackline copies of the preliminary plat which shall be drawn at a scale of one inch equals 100 feet. The preliminary plat shall be presented on a sheet or sheets 24 inches wide by 36 inches long within which are borders ½ inch wide on all sides. Where necessary, the plat may be on more than 1 sheet; provided that, if the plat is on more than 2 sheets, they shall be accompanied by an index sheet of the same size showing the entire subdivision. The plat shall show all proposals including the following:
(a) The information listed above under division (A)(2)(a) above, division (A)(2)(b) above, division (A)(2)(c) above (except that the data shall be shown only for those streets immediately adjacent to the tract), division (A)(2)(d) above, division (A)(2)(e) above, division (A)(2)(h) above, division (A)(2)(i) above (except that the data shall be shown only for those areas immediately adjacent to the tract), and division (A)(2)(k) above;
(b) Streets: Names, right-of-way widths; approximate grades and gradients; centerlines and lengths, and bearings between intersections with the centerlines of other streets, alleys, or tract boundary lines; similar data for alleys, if any;
(c) Other right-of-ways or easements: Location, width, and purpose;
(d) Dimensioned lot lines; lot and block numbers;
(e) Sites, if any, to be reserved or dedicated for public uses;
(f) Minimum building setback lines, if the information is required by the Planning Commission; and
(g) Site data: The number of lots; the minimum, typical and maximum lot area, and lot frontage; the acreage proposed for public uses, excluding streets, alleys and other right-of-ways and easements; the total number of residential dwelling units to be accommodated; and any other data deemed pertinent or as may be required by the Planning Commission. Total acreage of tract shall be shown.

(B) Profiles of ground surface, street grades; cross sections of grading, roadways, sidewalks; preliminary plans of sanitary and storm water sewer; modifications of requirements.
(1) The application for approval of the preliminary plat shall be accompanied by profiles showing existing ground surface and proposed street grades, including extensions for a reasonable distance beyond the limits of the proposed subdivision; typical cross-sections of the proposed grading, roadways, and sidewalks; and preliminary plans of proposed sanitary and storm water sewers or other storm drainage system (combination sewers shall not be acceptable) with grades and sizes indicated. All elevations shall be based on U.S.G.S. datum. All materials as are herein required shall be drawn to the scale and in the manner prescribed by the City Engineer.
(2) The City Engineer may modify these requirements in whole or in part; provided, however, that if it be done, the application shall be accompanied by a signed and dated written statement by him, which statement shall specifically describe the modifications made.
