§ 52.02 APPLICATION FOR ELECTRIC SERVICE; INSPECTION; CONNECTION; CUSTOMER SELECTION OF ELECTRIC SERVICE AND RATE SCHEDULE.
   Any person desiring to have premises connected with the electric distribution system of the company shall make application to the company. The application shall contain the name of the applicant, the lot and block number, the address, the kind of building, and the name of the owner or account. When the application has been properly filed, and deposit made as may be required, the company shall inspect the premises, and if they conform with the rules and regulations of the company for distribution of electric current, then the application shall be approved and the installation made. Rates are normally established on a 12-month basis, and the customer having selected a rate adopted to his or her service may not change to another rate within a 12-month period, unless there is a substantial change in the character or conditions of his or her service. A new customer will be given reasonable opportunity, normally not to exceed 3-months time, to determine his or her service requirements before definitely selecting the most favorable rate.
