868.03 PERMIT APPLICATION.
   (a)   Each applicant for a permit shall present and file with the Fiscal Officer his or her signed application form, setting forth the trade name under which he, she or it intends to do business, a general description of the vehicle for which a permit is desired, and any other information required by the Fiscal Officer pertinent to the issuance of a permit.
   (b)   The applicant shall further furnish:
      (1)   A copy of the certificate of title of the trolley-shuttle vehicle to be operated.
      (2)   The name and copy of a valid operator's license of each person who will operate the trolley-shuttle vehicle, and, if a driver is hired subsequent to the issuance of the permit, the applicant shall provide the Fiscal Officer in writing with the name and copy of a valid operator's license of said person. If the vehicle is operated as a trolley-shuttle vehicle within the Municipality by a driver whose name and operator's license arc not on file as provided for herein, or if said driver is in violation of Section 868.07, the Council may revoke or suspend the business license of the company or person, or the permit for the trolley being so operated, pursuant to Section 868.09.
      (3)   A policy or policies of liability insurance issued for the duration of the permit applied for, or longer, by a responsible insurance company, approved as to the reasonable sufficiency by the Mayor or his or her designee, which policy or policies shall provide indemnity for the protection to the applicant, and naming the Village as additional insured, against loss resulting from the operation of each trolley-shuttle for which a permit shall be issued, in the amount of one million dollars ($1,000,000) for injury or death of any one person in any one accident. The policy shall also contain underinsured/uninsured motorist coverage and a medical payment provision of at least five thousand dollars ($5,000). The applicant shall file with the Fiscal Officer a certificate of insurance showing said coverage. All insurance policies shall also contain a provision to notify the Village if the insurance is canceled.
      (4)   A copy of the Village business license, obtained in accordance with Section 802.02.
      (5)   A fee of twenty-five dollars ($25.00) per permit.
      (6)   Proof that each motor vehicle to be used as a trolley-shuttle vehicle passed an inspection by the State Highway Patrol, arranged by the owners and obtained at the owners' own expense. The vehicle inspection shalt verify that the vehicle has all of the safety equipment required by law and that the same is in safe and proper working order. Each vehicle used as a trolley-shuttle vehicle shall display a valid safety inspection sticker obtained as a result of the inspection by the Ohio State Patrol.
      (7)   The schedule and fixed route on which the trolley-shuttle vehicle will operate.
   (c)   The fixed route on which the trolley-shuttle vehicle will operate must be approved by a motion of the Council.
   (d)   The Mayor or his or her designee shall issue such permits as are necessary to cover the applicants who have applied for trolley-shuttle vehicle permits under this chapter, and who have conformed to the requirements of this chapter, including the approval of the fixed route by the Council, unless the Council, by resolution, shall limit the number of permits which can be issued, based upon the health, welfare, and safety of the public in the Village, or as provided in Section 868.04.
