(a) Upon the Mayor's or the Fiscal Officer's suspension of a taxicab driver's license that is not appealed to Council, or upon Council's suspension or revocation of any taxicab driver's license, such driver shall return to the Fiscal Officer the license and identification card theretofore issued to him; provided that, in the event such items have been lost, the applicant shall file an affidavit to that effect with the Fiscal Officer.
(b) Failure to file such affidavit or to return the license and identification card shall be punishable according to Section 866.99.
