(A) Pursuant to I.C. 16-46 (Board of Health fee schedules), the County Board of Health Department may, with the approval of the County Board of Commissioners, establish and collect fees for specific services and records as established by local ordinances and state law; provided, that such fees shall not be in excess of the costs of services rendered, and such fees shall be accounted for, and transferred to, the Health Fund of the county (1965 Acts), and amended effective May 15, 1988 to provide that no charge shall be made under I.C. 16-46 for each certificate of birth, death, or stillborn registration if the person applying for the certificate meets one of the following purposes:
(1) To establish the person’s age, or to establish the dependency of any member of the person’s family in connection with the person’s service in the Armed Forces of the United States;
(2) To establish the person’s age, or to establish the dependency of any member of the person’s family in connection with a death pension or disability pension of any person serving, or who has served, in the United States Armed Forces; or
(3) To establish or verify the age of a child in school who desires to secure a work permit (1988 Acts).
(B) The schedule of fees shall be as follows:
