No mobile food service permit will be issued by the city to an applicant unless and until the applicant satisfies the following conditions of issuance:
A. Provides to the city a copy of the applicant's approved "application for food or lodging establishment license" issued by the Oklahoma state department of health.
B. Provides to the city a copy of the applicant's current, valid Oklahoma sales tax permit.
C. An applicant for issuance of a permit under the status of a nonprofit, charitable or religious organization shall provide official written proof of exemption from the requirement of collecting sales tax in lieu of the requirement of a valid Oklahoma sales tax permit.
D. An applicant for issuance of a permit under the status of a nonprofit, charitable or religious organization shall not be relieved of the obligation to provide a copy of the applicant's approved "application for food or lodging establishment license" issued by the Oklahoma state department of health.
E. Provision to the city of a completed application for mobile food service permit on the application form prescribed by the city.
F. Payment of the prescribed fees to the city for processing the application for permit and for issuance of a mobile food service permit pursuant to an approved application as set forth in appendix A of this code.
G. Approval of the application for mobile food service permit by the city. (Ord. 2016-3, 11-1-2016)
