Duplicate issue of vehicle licenses may occur when the original license has become mutilated beyond serviceability, lost, stolen or for other proper cause. Whenever possible, the original license shall be surrendered upon duplicate issue. Application for duplicate issue shall be made on a special form provided for that purpose which shall contain a brief statement by the applicant citing the circumstances requiring need for reissue. The fee set out in section 10-1-7 of this chapter shall be charged for duplicate issue unless it is clearly established that the need for reissue is no fault of the applicant, at which time no fee shall be charged.
