3-10-4: PERMIT TO OPERATE:
   A.   Permit Required; Exception: An annual permit to operate a swimming pool is required for all swimming pools except for pools in connection with a building in R-1 zoning district and available to the family of the household and his private guests.
   B.   Annual Operation Permit Review: The application shall be made in writing to the Prospect Heights health department. An operation permit shall be renewed annually in May or whenever a pool is to be reopened for use after being closed down for a period no longer than one month or after any maintenance that may alter the structure, the equipment, the surfaces or operation of the pool that might also affect the health of the users.
   C.   Reinstatement: Public pools closed for a period of one year or longer shall obtain approval for opening from the state department of public health. A copy of such approval letter shall accompany the application to the Prospect Heights health department for a permit to operate.
   D.   Inspection Schedule: Representatives of the city health department shall make inspections of swimming pools before the annual permit to operate is granted. An additional inspection shall take place within two (2) weeks after the swimming pool is put into operation.
   E.   Private Pool Inspection Fee: Subsequent inspection of pools shall be made as desired by the Prospect Heights health department. Inspection of swimming pools at private residences shall be made upon request of the pool owner or operator and then only after payment of a twenty five dollar ($25.00) inspection fee.
