Any person desiring to engage in the occupation of peddler or solicitor shall apply to the City Clerk for a Certificate of Registration. Such application shall be made on a form provided by the Clerk and shall show, and contain in addition to such other information as the Clerk may desire, the following:
(a) Name and description of applicant.
(b) Permanent home address and local address of applicant.
(c) If employed, the name and address of employer.
