§ 90.22 ALARM DISPATCH RECORDS.
   The police responding to a dispatch call resulting from a burglary, robbery, personal hostage or robbery alarm notification shall record information as necessary to permit the Chief to maintain records, including, but not limited, to the following information:
   (A)   Identification of the permit holder;
   (B)   Identification of the alarm site;
   (C)   Arrival time and dispatch received time;
   (D)   Time of day and date;
   (E)   Weather conditions;
   (F)   Area and subarea (business; residential);
   (G)   Name of the permit holder's representative on premises, if any; and
   (H)   The responding police officer shall indicate on this dispatch record whether the alarm notification was caused by a criminal offense.
