1282.02   CONDITIONAL USE PERMIT REQUIRED; APPLICATION; ISSUANCE OR DENIAL.
   (a)   No person shall construct, erect or move an antenna without first obtaining a conditional use permit from the Board of Zoning Appeals as required by Section 1264.05.
   (b)   Application for such a permit shall:
      (1)   Contain a site plan indicating the parcel number, address and the applicant's name along with the antenna type and its location with reference to lot lines, building set back and other existing structures.
      (2)   Include a copy of any required FCC license.
      (3)   Include the manufacturer's specifications or other substantiating evidence to determine the antenna's strength, stability, surface area, dimensions and capability to withstand a wind of 100 miles per hour.
      (4)   Provide details of support, guys, braces and method of attachment.
      (5)   Provide a method of grounding.
      (6)   Provide such other information as is necessary to determine compliance with this section.
      (7)   Be accompanied by the application fee set forth in Section 1262.04(j).
      (8)   Be accompanied by a written signed statement of the proposed installer indicating his or her knowledge of the requirements of this section and his or her intention to comply therewith.
   (c)   The Board shall hold a hearing and shall consider comments of contiguous owners and other owners of visually affected lots in the determination of the permit issuance or denial.
