§ 151.37 PERMIT APPLICATION.
   Application to the town for a street and driveway access permit shall be made on a form provided by the town. All required signatures or seals of approval shall be shown on the permit application prior to submission. Fees established by the town shall be paid upon submission of a signed permit application.
