All applicants for privilege licenses required by this subchapter shall make a formal application setting out in such application:

(A) The number of cabs then in operation;

(B) The number of additional taxicabs requested;
(2003 Code, § 24-2.02.2)
(C) The make, year, and model of cabs then in use and those requested to be placed in use;
(2003 Code, § 24-2.02.3)
(D) The name of the proposed driver; and
(2003 Code, § 24-2.02.4)
(E) Such further information as the Board may request.
