§ 111.16 APPLICATION FOR PERMIT.
   Application for a permit to peddle or solicit shall be sworn to and addressed to the office of the Town Clerk, who shall have the authority to require any or all of the following information:

   (A)   Name of organization applying for a permit and the address of its headquarters;

   (B)   Names and addresses of its principal officers and management;
(2003 Code, § 3-202.2)
   (C)   The purpose for which such peddling or solicitation is to be made and the use to be made of any receipts resulting therefrom;

   (D)   The name of the persons by whom the receipts of such peddling or solicitation shall be disbursed;

   (E)   The name and address of the persons who will be in direct charge of conducting the peddling or solicitation;

   (F)   An outline of the methods to be used in conducting the peddling or solicitation;
(2003 Code, § 3-202.6)
   (G)   The time when such peddling or solicitation shall be made, giving the preferred dates for the beginning and ending of such peddling or solicitation;

   (H)   A full statement of the character and extent of the work being done by the applicant within the town and such other information as may be required by the office of the Town Clerk to determine fully the kind and character of the proposed peddling or solicitation and whether such peddling or solicitation is in the general interest of the public; and/or

   (I)   A statement to the effect that if a permit is granted it will not be used or represented in any way as an endorsement by the town.
