§ 117.03 LICENSING.
   (A)   No person shall perform massage and bodywork therapy unless such a person shall have first applied for and received a privilege license from the city-county Tax Collector. Such privilege license shall be issued only to those persons meeting the definition of health massage/bodywork therapist defined in § 117.02 above.
   (B)   The application for the license required by this section shall be upon a form approved by and filed with the city-county Tax Collector. Such application shall include the following information:
      (1)   The name and business address of the applicant; and
      (2)   A diploma or certificate of graduation from an institute or school of massage and bodywork therapy whose curriculum is licensed under a state licensing requirement, or a valid document of certification from the American Massage Therapy Association or the National Certification Board for Therapeutic Massage and Body Work, or the information required in division (D) of this section.
   (C)   Each applicant for a license issued pursuant to this section shall pay the privilege license tax provided in this code.
   (D)   Requirements for licensure. Under North Carolina State Law, G.S. Chapter 90, Article 36, the requirements for licensure are as follows:
      (1)   Has obtained a high school diploma or equivalent;
      (2)   Is 18 years of age or older;
      (3)   Is of good moral character as determined by the Board;
      (4)   Has successfully completed a training program consisting of a minimum of 500 in-class hours of supervised instruction at a Board-approved school;
      (5)   Has passed a competency assessment examination that meets generally accepted psychometric principles and standards and is approved by the Board; and
      (6)   Has submitted fingerprint cards in a form acceptable to the Board at the time the license application is filed and consented to a criminal history record check by the North Carolina Department of Public Safety.
