(A) No public utility employee shall remove a manhole cover and enter such manhole for any purpose without another employee or a public officer guarding the manhole.
(B) No public utility supervisor shall order an employee to open and enter a manhole which is unguarded, or at any time or place when a rope of life line is not attached to the person entering the manhole.
