A. Emergency vehicle permits shall not be transferable to any other person, company or vehicle.
B. In the event of a name change, not involving a change of an address of the permittee, the emergency vehicle permit shall be returned to the director for cancellation and a new emergency vehicle permit application form shall be submitted. A new emergency vehicle permit for the unexpired term will be issued.
C. In the event the vehicle for which the emergency vehicle permit is issued is disposed of, the emergency vehicle permit shall be removed from the vehicle and the emergency vehicle permit returned to the director for cancellation.
D. In the event of loss, destruction or mutilation of the emergency vehicle permit, the permittee to whom it was issued may obtain a duplicate upon application to the director.
