9.08.060   Application for emergency vehicle permit.
   A.   An application for an emergency vehicle permit shall be made to the director.
   B.   The application for an emergency vehicle permit shall be made upon a form furnished by the director. It shall contain such information concerning the applicant's background and experience as the director prescribes, in addition to other information required by law.
