2.16.020   Duty to file statement—Exceptions.
   A.   Every public officer shall file with the clerk of the board of supervisors a sworn financial disclosure statement covering the previous calendar year, on or before January 31 of each year. Every person appointed to a public office shall file a sworn financial disclosure statement within fifteen days of such appointment.
   B.   Candidates for public office shall file with the election filing officer a sworn financial disclosure statement covering the preceding twelve-month period at the time of the filing of the nominating petitions.
   C.   The statement shall contain:
      1.   The name and address of the public officer, the name of each member of the household, and all names under which they do business;
      2.   Identification of each employer and of each source of compensation amounting to more than one thousand dollars annually received by the public officer and the members of the household in their own names, or by any other person for the use or benefit of the public officer and members of the household, a brief description of the nature of the services for which the compensation was received. The disclosure of information privileged by law and the disclosure of individual items of compensation that constitute portions of the gross income of a business or profession from which the public officer and the members of the household derive compensation is not required.
      3.   The name of every corporation, trust, business trust, partnership or association in which the public officer and members of the household, or any other person for the use or benefit of the public officer and members of the household, have an investment or holdings of over one thousand dollars at fair market value as of the date of the statement, or in which the public officer or a member of the household holds any office or has a fiduciary relationship, and a value and a description of the financial interest, office or relationship. The disclosure of the name of any bank or other financial institution with which the public officer or member of the household has a deposit or withdrawal share account is not required.
      4.   All real property interests in the state, including street address, specific location and approximate size or legal description, to which the public officer or members of the household holds legal title, or a beneficial interest in, excluding their residences and property used primarily for personal recreation by the public officer or members of the household and the value of such interest. If a real property interest was acquired or divested during the preceding calendar year, the transaction and the date of occurrence shall also be identified.
      5.   The name and address of any person to whom the public officer and members of the household owe more than one thousand dollars and the name of any person who owes the public officer and members of the household more than one thousand dollars. The disclosure of debts resulting from the ordinary conduct of a business or profession, debts on the residence of the public officer or members of the household, debts arising out of secured transactions for the purchase of consumer goods, and debts secured by cash values on life insurance is not required.
      6.   The identification of any account receivable exceeding one thousand dollars held by the public officer or member of the household in their own names, or by any other person for the use or benefit of the public officer or members of the household. The disclosure of information privileged by law and of debts arising from the ordinary conduct of business or profession is not required.
      7.   The source of each gift, or accumulated gifts from a single source, of more than five hundred dollars received by the public officer and members of the household in their own names during the preceding twelve months, or by any other person for the use or benefit of the public officer and members of the household. The disclosure of gifts received by inheritance, or by distribution from a trust established by a spouse or an ancestor, or from relatives is not required. Political campaign contributions are not gifts.
      8.   A description of any professional occupation or business license in which a public officer or member of the household has an interest, issued by the county or by a state department, agency, commission, institution or instrumentality which requires for its issuance the consideration of the application by the board of supervisors, including the name in which the license is issued, the type of business or profession, and its location.
      9.   A list of any bonds issued by the county held by a public officer or member of the household at any time during the preceding calendar year in excess of one thousand dollars. If a reportable county bond was acquired or divested during the preceding calendar year, the transaction and the date of occurrence shall also be identified.
