A. The department shall assess and collect fees for the review and approval of plans for potable water systems, wastewater collection systems, or on-site disposal systems. The fees are required when an application is submitted, and are not refundable.
B. The department shall assess a flat rate fee for the following services:
1. Site plan review and approval required pursuant to ARS § 45-596 for notice of intention to drill a well, twenty-five dollars; with site visit, one hundred fifty dollars.
2. Approval of Sanitary Facilities for Subdivisions. Approvals are granted in phases of 150 lots or less.
a. Sewage treatment and disposal is provided outside the boundaries of the individual lots, 150 lots or less, three hundred dollars;
b. Sewage treatment and disposal will be located within the boundary of the lot, 40 lots or less, five hundred dollars; 41 to 150 lots, one thousand dollars;
c. The appropriate fee(s) specified in B.2.a. and B.2.b. applies to each phase if a subdivision includes more than 150 lots.
3. Public Water Systems; Approval To Construct/Approval Of Construction.
a. Approval to construct/approval of construction fees established in Table 1 plus any of the following:
i. A site visit to verify a construction deviation, one hundred fifty dollars per site visit.
ii. A site visit to conduct sampling, one hundred fifty dollars per visit.
iii. A site visit to witness a pressure test, one hundred fifty dollars per visit.
b. If a water project includes elements from more than one fee category in Table 1, the applicant shall pay the sum of the fees established in Table 1 and any additional fee specified in subsections B.3.a.i., B.3.a.ii. and B.4.a.iii.
c. Issuance of plan review exemption letter, fifty dollars.
4. Wastewater Collection System/ On-site Disposal System; Verification of Conformance with General Permit.
a. New permit and expansion fees established in Table 1 plus any of the following:
i. A request for an alternative design, installation, or operational feature, seventy-five dollars per change.
ii. A design requiring an interceptor, one hundred dollars per interceptor.
iii. A site visit.
a.) To verify a construction deviation, one hundred fifty dollars per site visit.
b.) To verify soil evaluation/site plan, one hundred fifty dollars per visit.
c.) To perform a final inspection, one hundred thirty dollars per site visit.
b. If an on-site wastewater treatment system is based on a design that combines elements from more than one type 4 general permit, the applicant shall pay the fee established in Table 1 for the appropriate type 4 general permit plus one hundred dollars for each additional general permit used in the design, and any additional fee specified in subsections B.4.a.i., B.4.a.ii., and B.4.a.iii.
c. Presale inspection and transfer of ownership, fifty dollars per transfer for an on-site disposal system operating under a type 1 or type 4 general permit.
d. Septic system location, ten dollars per request.
e. Issuance of a plan review exemption letter, fifty dollars.
C. In addition to the fees established by this section, any other site visit, one hundred fifty dollars.
D. If inspection determines violations have occurred, an investigation fee of up to one thousand dollars may be assessed to recoup costs of additional inspection and compliance activities.
E. The department shall not review a request for a plan review if the initial fee established has not been paid.
