The application for the license required by this chapter shall be made on a form prescribed by the Village Police Department. The application shall include the name of the person making the application, the address of the location of the sale, and the beginning and ending dates of such sale. The license issued by this chapter shall be valid only for the property located at the residence address included in the license application. The license shall also provide that any such sale or temporary business activity which is canceled because of rain, other weather conditions, or conditions beyond the control of the license holder, may be rescheduled without further license application for a three day period occurring not later than thirty days after the beginning date of the sale or activity contained on the license application form, upon written notice to the Police Department.
