Any person, firm, or corporation desiring a license for the use or operation of such devices, shall file an application therefor with the Village Clerk, upon a form provided by him, setting forth the name and address of the applicant; the name of the owner of such device; the date upon which it is intended to be used; and such other information as may be prescribed.
