In addition to the information and statements required under Section 808.03, and at the same time such information and statements are submitted, the applicant shall submit the following information and statements:
(a) A statement that the applicant is not disqualified from receiving a license by reason of any matter or thing contained in this chapter, State law, laws of the United States or ordinances of the Village; and
(b) A statement that the applicant will not violate any of the ordinances of the Village in the conduct of his or her business.
