808.03 LICENSE APPLICATION; APPLICATION FEE AND CHARGE FOR STATE POLICE CHECK.
   (a)   Application for the license required by Section 808.02 shall be made to the Mayor, in writing, signed by the applicant, if an individual, and signed by all the partners, if a partnership, or by a duly authorized agent thereof, if a club or corporation, verified by oath or affidavit. Such application shall contain all information and statements required by the State for a similar application for a State license to sell alcoholic liquor at retail as set forth in the Illinois Liquor Control Law, approved January 31, 1934, including all amendments and additions thereto.

   (b)   In addition to the requirements provided for in subsection (a) hereof, each application shall be accompanied by a nonrefundable application fee of fifty dollars ($50.00) and an additional amount sufficient to cover all costs incurred by the Village in having the applicant checked by the State Police.
