(A) A permit shall be required for the construction of any driveway connecting private property to a public street.
(B) A request for such a permit shall be delivered to the city clerk and shall include the street address of the property involved, a drawing showing the size and location of the driveway, the use of the property on which the driveway is proposed to be constructed, and the type and number of motor vehicles expected to use such driveway.
(C) The permit fee for a driveway shall be set in accordance with the schedule in § 150.11(A) of this Code.
