Application for the permit required by Section 905.06 shall be made on a form, giving such facts concerning the excavation as required by the Director of Public Service and shall be confined to a single project. The permit shall be issued by the Director after a cash deposit or a bond with good and sufficient sureties to cover the cost of restoration as estimated by the Director or an undertaking by a utility company has been posted with the Clerk-Auditor.
