10-5: HEALTH AND DENTAL INSURANCE UPON RETIREMENT:
   A.   COBRA: As stated in the benefits continuation (COBRA) policy in this manual, retirees of Ogden City may elect to continue group health and dental insurance plan benefits and coverage for a period of up to eighteen (18) months under the same coverages, terms and conditions they had immediately prior to retiring.
   B.   Premium Costs: Under COBRA, retirees of the City are responsible for the monthly premium costs of applicable health and dental insurance plan coverage, including a two percent (2%) administrative fee and are required to timely pay applicable monthly premiums to the Personnel Office.
   C.   After Eighteen Months: After eighteen (18) months under COBRA extended group health and dental insurance plan benefit coverage, Ogden City retirees may elect, at their expense, to extend the insurance benefits available through the City sponsored group plan(s) under the same conditions and terms available to other employees at a monthly premium cost to the retiree. The exact cost of the premium amount is determined annually through negotiated agreement by the City with the plan provider. (Eff. 6-1-1996)
