   A.   Per Occurrence Event:
      1.   Minimal Preventability: Disciplinary action recommended for a preventable accident classified as "minimal" is a minimum of a verbal discussion and/or warning that is documented in the employee's personnel file or a formal written warning/caution documented in the official personnel file which is maintained in the personnel office.
      2.   Moderate Preventability: Disciplinary action recommended for a preventable accident classified as "moderate" is a minimum of a written warning/caution that is documented in the employee's official personnel file which is maintained in the personnel office, up to a maximum of one day suspension without pay.
      3.   Substantial Preventability: Disciplinary action recommended for a preventable accident classified as "substantial" is a minimum of a one day suspension without pay documented in the employee's official personnel file which is maintained in the personnel office, up to a maximum of a recommendation for termination.
   B.   Additional Event: All other documented disciplinary action or determination of extenuating circumstances should be considered by the department and follow the progressive disciplinary action guidelines. All disciplinary actions require written documentation in the employee's official personnel file which is maintained in the personnel office. (Eff. 3-14-2007)
