The following are the procedures for work related accident/illness requiring medical treatment:
A. Employees injured on the job requiring medical treatment are requested to report to the WorkMed facility located at 1355 West 3400 South (by the Ogden Hinckley Airport). Hours are weekdays from seven thirty o'clock (7:30) A.M. to five thirty o'clock (5:30) P.M. (WorkMed phone number: 627-1273).
B. For minor injuries occurring on the job after WorkMed hours, report to WorkMed during their business hours or call to schedule an appointment.
C. For minor injuries occurring on the job after WorkMed hours and seriousness of condition is questionable, call the McKay-Dee Hospital emergency room number 625-2020 to discuss injury and receive further instructions. Do not go to hospital emergency room unless directed to do so by emergency room person.
D. In urgent situations call WorkMed at 627-1273, during their business hours or the McKay-Dee Hospital emergency room number 625-2020 if after WorkMed hours to receive instructions.
E. Life threatening accident/illness requiring emergency treatment during WorkMed hours, should report directly to McKay- Dee Hospital emergency room at 3939 Harrison Boulevard, or the nearest emergency facility. If seriousness is questionable call WorkMed, 627-1273, for instructions to proceed to emergency room or WorkMed facility. Always notify emergency room staff that patient is an employee of Ogden City and WorkMed is Ogden City's designated medical facility.
F. Life threatening accident/illness requiring emergency treatment after WorkMed hours should report directly to McKay-Dee Hospital emergency room at 3939 Harrison Boulevard or nearest emergency facility. Always notify emergency room staff that patient is an employee of Ogden City and WorkMed is Ogden City's designated medical facility. (Eff. 3-14-2007)
