All applications approved by the Examining Board shall be forthwith filed with the City, and all applications rejected shall be so filed together with a written statement disclosing the reasons for the rejection. Every person who shall receive a license, card or badge, either as a master plumber, journeyman plumber or apprentice, shall register with the secretary of the Housing Renewal Commission, upon such form or forms as the Commission may prescribe, his/her name, place of business, home address, length of time engaged in the occupation of plumbing and where he/she served his/her apprenticeship.
