A permit for a public sale or garage sale shall be issued by the City Manager or a person acting and authorized to act on his/her behalf upon the applicant's complying with the following provisions:
(a) Giving his/her name and permanent address;
(b) Obtaining the permit required by this chapter;
(c) Giving the name and address of the party for whom the sale is being held; and
(d) Giving the date of sales and number of sales in a calendar year.
