The inspection charges as defined in Section 1735.01 shall be paid monthly, on or before the tenth day of the month following the month for which the charge is made. Payment shall in every case, be accompanied by a statement on a form to be furnished by the Health Commissioner setting forth such data and information as is required. Overdue payments shall be subject to a penalty of five percent.
