(a) Application for a public dance, public dance hall, or roller skating rink or arena permit required by Section 747.02, including the renewal of a license, shall be made in writing and sworn to on a form provided by the Safety-Service Director and pursuant to the terms of this Chapter. A public dance, public dance hall, or roller skating rink or arena permit shall only be issued to an owner, or a business, or their designated agent who has provided a current valid street mailing address to the Safety-Service Director for the receipt of legal notices from the City. A post office box or similar location may not be used for legal mailings.
(b) The application for a permit to operate a public dance, public dance hall, or roller skating rink or arena shall include the following information:
(1) A list identifying every individual who will be directly engaged in managing or supervising the daily operations of the facility, and for each individual, the individual's name and address.
(2) The business history of the applicant regarding any government-issued licenses or permits related to the operation of any public dance, public dance hall, roller skating rink or arena, including any currently held by the applicant and any previously issued licenses or permits that were revoked or suspended and the reasons therefor;
(3) If the applicant is a business entity that is required to register with the Secretary of State of Ohio, a copy of a current certificate of good standing issued by that office.
(c) Each application shall include a filing fee of one of the following, as applicable:
(1) For a temporary permit, the fee shall be two hundred and fifty dollars ($250.00). The applicant shall specify the hours during which the dance or similar event shall operate, which shall not exceed eight hours in duration.
(2) For public dance halls, skating rinks or arenas or similar entertainment that is operated on a permanent, continuous or year-round basis, the fee shall be one thousand and five hundred dollars ($1,500.00) per annum. Annual permits shall be effective from January 1 to December 31 of each calendar year.
(d) Any person who knowingly makes a false license application or who procures or seeks to procure a license for another shall be guilty of a misdemeanor of the first degree.
