The fee for a permit authorizing vehicles to exceed specified weight limits, as set forth in Section 440.12 of the Traffic Code, shall be:
(a) Thirty dollars ($30.00) per individual trip (one unit to one destination).
(b) Fifty dollars ($50.00) per trip and return (one unit to one destination).
(c) One hundred fifty dollars ($150.00) quarterly consisting of five trips total (one unit to one destination).
(d) Three hundred dollars ($300.00) annually consisting of ten trips total (one unit to one destination).
