§ 156.061 APPLICATION.
   (A)   An application for a final plat shall be submitted to the Planning and Zoning Department at least three weeks prior to the Planning and Zoning Commission meeting at which the plat is to be heard.
   (B)   The following shall be submitted with the application:
      (1)   Application fee;
      (2)   Three photographic mylar (or similar material) copies of the plat submitted after City Council approval of said final plat);
      (3)   Twelve paper copies at one inch equals 100 feet scale of the plat, one 11"x 17" legible copy and one electronic copy of the subdivision;
      (4)   Statement listing all owners of record of the land being subdivided;
      (5)   Statement listing all waivers being sought in accordance with § 156.007;
      (6)   Certified as built construction plans and profiles of all streets, alleys, sanitary and storm sewers, water mains, sidewalks, subdivision grading and other appurtenances in accordance with §§ 156.150 through 156.160;
      (7)   Protective covenants or restrictions to be imposed upon the subdivision;
      (8)   Performance and/or maintenance bonds in accordance with §§ 156.150 through 156.160;
      (9)   A list providing the property address, owner and their mailing address of all properties within 200 feet of the exterior boundaries of the plat, obtained from the County Recorder’s office; and
      (10)   Any other supplemental material as specified in this chapter.
