1741.02  APPLICATION; CONTENTS.
   No person shall construct, alter or add to any building unless he shall have first obtained from the Recorder a permit, the application for which shall provide a listing of the following:
   (a)    A description of the land upon which the proposed building is to be erected or work is to be done, either by lot, block or tract or by a similar general description that will readily identify and definitely locate the property, building or work.
   (b)    The purposes for which such building will be used when so erected or constructed, or in the case of alteration or repair of any such existing building or structure, the uses and purposes for which the same will be devoted after such repair and alteration.
   (c)    Copies of plans and specifications of the proposed building to be constructed or erected, or in the case of repair and alteration, plans and specifications of the proposed alterations and repairs; or in either instance, if plans and specifications are not available, a full and distinct statement as to the character, type and size of the building to be so constructed or erected, or a full and distinct statement of the character; type and extent of the proposed alterations and repairs, as the case may be.
   (d)    If such application is for a permit to move any such building or structure, such application shall distinctly and fully set forth the existing location of the lot, tract or property upon which such building or structure is located and a similar description of the lot, tract or property to which such building will be moved; and when the application is for a permit to demolish an existing building or structure, such application shall plainly set forth the manner of such demolition and further state when disposition is to be made of the materials resulting therefrom.
   (e)    If any applicant for a building permit be a contractor, then such applicant must furnish proof with his application of current valid contractor's licenses issued both by the state of West Virginia and the city of New Martinsville.
   (f)    If the plans and specifications of proposed construction, alteration or addition show work of an electrical nature to be performed, then the applicant for permit must furnish proof that such work will be performed by a person duly and validly licensed as an electrician by the state of West Virginia, except a person who performs electrical work with respect to any property owned or lease by such person.
   (g)    In accepting a permit from the building and safety inspector, the applicant shall agree to indemnify and hold the City harmless against all claims, warrants, demands, causes of action or suits arising from or occasioned by the work which shall be the subject of the permit.
