(a) Any contractor, company, individual, utility or entity who plans to excavate on any public street or highway, or who plans by said excavation to connect into a public utility shall first obtain from the City Clerk a permit to excavate at least three days in advance of the start of excavation. The contractor, company, individual, utility, or entity shall obtain the permit by applying to the City Clerk. The application shall include:
(1) The name, address and telephone number of the person who will be doing the excavation;
(2) The exact location with a diagram of the location to be excavated;
(3) The length of time until completion and the nature of the excavation;
(4) Whether any other utilities have been contacted in regard to said excavation;
(5) The person on the job site who can be contacted in regard to the excavation; and
(6) The estimated cost of the excavation.
(b) There shall be no fee for this permit. The City Clerk may, with the approval of the Chairman of the Street Committee, for extenuating circumstances waive the three day notice provision and immediately grant said permit.
