(A) The name and address, including the telephone number, of the legal owner or agent of the property, and a citation of the last instrument conveying title to each parcel of property involved in the proposed subdivision, giving grantor, grantee, date, and land records reference.
(B) The citation of any existing legal rights-of-way or easements affecting the property.
(C) Existing covenants on the property, if any.
(D) The name and address, including the telephone number, of the professional person(s) responsible for subdivision design, for the design of public improvements, and for the land survey.
