The application for building permit shall state at the minimum the applicant's name, address and phone number; the contractor's license number and phone number; the owner's address and phone number; the property location; the type of improvement, proposed use, structural size, height, length and width and the estimated value of proposed building or improvement; if there are any easements; the primary construction materials; whether the building will use a septic tank and whether the building is in a floodplain; the cost of the sewer permit, water permit and building permit; provide a sketch of the lot indicating lot size, shape, existing structures, alleys or easements and adjacent streets; provide a sketch, plan or plat of the proposed structure indicating sizes; and shall be signed and dated by the owner and contractor. If evidence of property lines is not visible creating the possibility of a dispute regarding the location of property lines, the building inspector may require a survey prior to the approval of the building permit. Said application for building permit shall be on file in the office of the city clerk, and may be amended from time to time by the city council of the city of Nauvoo, Illinois.
