An applicant for a license or renewal thereof from the city of Nauvoo shall be made to the mayor as the liquor control commissioner in and for the city and shall be in writing, signed by the applicant or the duly authorized agent, under oath, and shall contain the following information and statements:
A. The name, age and address of the applicant in the case of an individual; in the case of a copartnership, the person entitled to share in the profits thereof; and in the case of a corporation for profit, or club, the date of incorporation, the objects for which it was organized, the names and addresses of the officers and directors, and if a majority in interest of the stock of such corporation is owned by one person or his nominee, the name and address of such person and his nominee and the name and address of the registered agent for the corporation for the state of Illinois and a certificate of good standing from the Illinois secretary of state's office;
B. The citizenship of the applicant, his place of birth and if a naturalized citizen, the time and place of his naturalization;
C. The amount of goods, wares and merchandise on hand at the time application is made;
D. The location and description of the premises or place of business which is to be operated under such license;
E. A statement that the applicant in the case of an individual; in the case of copartnership, the person entitled to share in the profit thereof; or in the case of a corporation for profit, or a club, the officers and directors and majority shareholders have never been convicted of a felony and are not disqualified to receive a license by reason of any manner or thing contained in this code;
F. Whether a previous license by any state or subdivision thereof, or by the federal government has been revoked, and the reasons therefor;
G. The applicant will not violate any of the laws of the state of Illinois or the United States or any provision of this chapter or provisions of this code;
H. Whether the application is for an original or renewal license.
If the application is made on behalf of a partnership, firm or association, club or corporation, then the application shall be signed under oath by at least two (2) members of such partnership, firm or association or club or the present secretary of such corporation. (Ord. 1994-3, 8-9-1994)
