§ 91.087 APPLICATION.
   Applications for excavation and tunnel permits shall be submitted to the City Clerk, and shall describe the location of the intended excavation or tunnel, the size thereof, the purpose therefor, the person doing the actual excavating work and the name of the person for whom the work is being done, and shall contain an agreement that the applicant will comply with all ordinances and laws relating to the work to be done. Applications shall be in the form prescribed by the Superintendents of Utilities and Streets. If approved, the permit shall be issued by the Superintendents of Utilities and Streets, or the Superintendents’ designated representative.
