§ 91.017 FORMS.
   (A)   (1)   The forms shall be of wood or metal, straight and free from warp, and of sufficient strength to resist springing during the process of depositing the concrete against them. Wood forms shall consist of two-inch surface planks.
      (2)   Metal forms shall be of approved section and shall have a flat surface on top. They shall be so designed that divider plates or other devices for holding the form in place will not cause planes of weakness in the concrete and subsequent cracking.
   (B)   (1)   The forms shall be of a depth equal to the depth of the curbing, and so designated as to permit of secure fastening together at the tops.
      (2)   They shall be securely staked, braced, held firmly to the required line and grade, and shall be sufficiently tight to prevent leakage of mortar.
      (3)   All forms shall be cleaned thoroughly and oiled before concrete is placed against them.
