§ 114.22 APPLICATION.
   Any person desiring a permit required by this chapter shall make written application to the Chief of Police. The application shall state or contain the following:
   (A)   A statement as to whether or not the applicant has been convicted of any crime, misdemeanor or violation of any state or federal law or municipal ordinance or code; the nature of the offense; the punishment or penalty assessed therefor, if previously convicted; and the place of conviction;
   (B)   Whether the applicant, upon the sale or order, shall demand, accept or receive payment or deposit of money in advance of final delivery;
   (C)   The period of time the applicant wishes to engage in business within this city;
   (D)   Name of applicant, date of birth, social security number and driver’s license number of applicant;
   (E)   The local and/or permanent address of the applicant;
   (F)   The local and/or permanent address of the firm, person or persons he or she represents;
   (G)   The kind of goods, wares, merchandise or services which the applicant offers for sale or exhibit;
   (H)   Current copy of the sales tax permit issued by the Comptroller of Public Accounts of the State of Texas;
   (I)   If the applicant is a corporation, it shall file with its application a certified copy of its charter, and if it is not incorporated under the laws of the State of Texas, a certified copy of its present permit to do business in the State of Texas;
   (J)   The last five cities or towns wherein the applicant has worked before coming to this city;
   (K)   Such other relevant information as may be required for the investigation of the applicant;
   (L)   In case the applicant desires to sell meat, fish or seafood products, or other food products, the applicant shall comply with these provisions, as well as those set forth in Ch. 96 of the code (Food and Food Establishments).
   (M)   A physical address of the location where the event is to take place.
   (N)   The name, address and phone number of the person who owns the site where the event is to take place.
