§ 111.162  LICENSE.
   (A)   It shall be unlawful to operate a bed and breakfast establishment within the corporate limits of the village without having a license from the village. Application for a license to operate a bed and breakfast establishment shall be made to the Village Clerk, in writing on a form to be furnished by the Village Clerk.
   (B)   The application shall be verified by oath or affirmation shall contain the following information:
      (1)   The name, age and address of the applicant;
      (2)   The location and description of the bed and breakfast establishment which is to be operated under the license; and
      (3)   Whether the applicant has made similar application for a similar license on premises other than described in the application and whether such application was granted or denied.
