§ 52.053 WASTEWATER DISCHARGE PERMIT APPLICATION CONTENTS.
   (A)   All users required to obtain a wastewater discharge permit must submit a permit application. The City Manager may require all users to submit as part of an application the following information:
      (1)   All information required by § 52.090(B) of this ordinance;
      (2)   Description of activities, facilities, and plant processes on the premises, including a list of all raw materials and chemicals used or stored at the facility which are, or could accidentally or intentionally be, discharged to the POTW;
      (3)   Number and type of employees, hours of operation, and proposed or actual hours of operation including monthly and seasonal variations;
      (4)   Each product produced by type, amount, process or processes, and rate of production;
      (5)   Type and amount of raw materials processed (average and maximum per day);
      (6)   Site plans, floor plans, mechanical and plumbing plans, and details to show all sewers, floor drains, and appurtenances by size, location, and elevation, and all points of discharge showing all pipe sizes, manholes and location of sanitary and storm sewer leaving the building or premises, together with all connections to lateral sanitary and storm sewers;
      (7)   Time and duration of discharges;
      (8)   SIC Code of the operation(s) carried out by the user.
      (9)   Wastewater constituents and characteristics including, but not limited to, those set forth in § 52.016 of this ordinance as determined by a reliable analytical laboratory; sampling and analysis shall be performed in accordance with 40 C.F.R. Part 136;
      (10)   Proposed or actual hours of operation of pretreatment system and the name of the IEPA certified pretreatment operator;
      (11)   Significant Industrial User’s source of intake water together with the types of usage and disposal sources of water and the estimated volumes in each category; and
      (12)   Any other information as may be deemed necessary by the City Manager to evaluate the wastewater discharge permit application.
   (B)   Incomplete or inaccurate applications will not be processed and will be returned to the user for revision.
