921.22 VIOLATION TYPES.
   (a)   Violations Leading to Warning Letters, Fines, Hearings, and/or Permit Suspensions. These following violations are serious enough that they cannot be allowed to continue. They will result in warning letters, hearings, and fines as necessary to cause the haulers to stop them. Continued violations of this nature can lead to denial or suspension of the Village HWDP. This list is intended to inform, but is not to be considered all-inclusive:
      (1)   Delivery of wastewater whose strength exceeds the critical limits set forth in the HWDP for that hauler.
      (2)   Delivery of wastewater that causes the Wastewater Treatment Plant to violate any applicable effluent limitations or standards, water quality standards or any legally applicable requirement, including court orders or State or Federal statutes, rules, regulations or orders.
      (3)   Failure to comply with any of the disposal rules and procedures established herein.
      (4)   Failure to complete manifests.
      (5)   Failure to submit manifests.
      (6)   Improper use of SRF equipment.
      (7)   Failure to properly maintain the facility.
      (8)   Failure to clean up sample bottles or spills in the facility.
   (b)   Critical violations are so serious that even one violation can lead to permit denial or suspension of the Village HWDP. The following list of violations is intended to illustrate the sort of violations that are critical, cannot be tolerated, and which may lead to immediate suspension or denial of the HWDP. This list is intended to inform, but is not to be considered all-inclusive:
      (1)   Discharge of wastewaters prohibited herein.
      (2)   Discharge of wastewater other than listed on the HWDP.
      (3)   Falsification of manifest or samples.
      (4)   Transfer of permit to a third party.
      (5)   Use of a vehicle that is not listed in the permit application.
      (6)   Use of a vehicle that does not correspond to the vehicle identification card.
      (7)   Vandalism.
      (8)   Misconduct toward a facility operator.
      (9)   Failure to display any document requested by the facility operator.
