921.08 PERMIT RENEWAL REQUIRED ANNUALLY,
   (a)   On October 1 of each year, a package for the required annual HWDP renewal shall be sent to the currently permitted haulers. The package will include:
      (1)   An HWDP renewal application form.
      (2)   A summary of the fees and fines for the firm's past year of activity.
      (3)   An updated copy of the “Rules and Procedures” manual.
      (4)   Instructions for the annual renewal application.
   (b)   The annual HWDP renewal application shall be sent no later than November 1 of each year to the Village of Mt. Gilead, Attention Village Administrator:
