(a) Each vehicle that is to be used for hauling wastewater should be listed on the HWDP application form. Additional sheets should be added as necessary to cover all vehicles needing permits.
(b) The HWDP application form requires:
(1) The name of the company.
(2) The owner's name or authorized agent.
(3) The company street address.
(4) The company telephone number.
(5) Complete information on vehicles that shall be used to deliver wastewater (make, model, year, license plate, state, and nominal tank capacity).
(6) A complete list of other permits held for the transportation and/or disposal of wastewater (permit number, licensing agency, and purpose of permit).
(7) The date of submittal.
(8) The signature of the owner or authorized agent.
