921.05 PERMIT APPLICATION REQUIREMENTS.
   (a)   Upon request, the permit candidate shall receive a package for HWDP application that includes:
      (1)   A letter stating the purpose of the application form.
      (2)   An HWDP application form.
      (3)   A list of documents to provide to the Village.
      (4)   A copy of the SRF procedures and fees.
   (b)   The completed HWDP application shall include:
      (1)   The completed HWDP application form.
      (2)   A copy of the Health Department License for wastewater transportation for each vehicle listed in the HWDP application form.
      (3)   A copy of any other permit held for the transportation or disposal of wastewaters.
      (4)   An insurance certificate for all vehicles listed in the HWDP application form.
      (5)   Two pictures of each vehicle showing the license plate number.
      (6)   A list and one picture I.D. of all drivers.
      (7)   A return envelope.
      (8)   A one-time fifty-dollar ($50.00) HWDP application fee for each vehicle.
