1311.08 SUBDIVISION OF THREE LOTS OR LESS.
   Plat, plans and data involving subdivisions of three lots or less shall include but not be limited to the following:
   (a)   The provisions of Sections 1311.02 to 1311.05 may be waived for subdivisions of three lots or less and the following requirements shall become applicable:
      (1)   The proposed plat shall be drawn at a scale of one inch equals 100 feet or greater, though not less than one inch equals 100 feet.
      (2)   The proposed plat shall be legibly drawn on linen or comparable permanent material of the size not to exceed thirty-one inches by forty-one inches.
      (3)   The plat shall show or be accompanied by the following:
         A.   Title block in the lower right corner to include:
            1.   Location by Municipality, County and State;
            2.   Names and addresses of the recorded owner or owners and notorized affidavit;
            3.   Names of Registered Professional Engineer or Registered Surveyor, if any, who surveyed the property and/or prepared the plat; and
            4.   North point, graphic scale and date of plat;
         B.   Lot lines, dimensions and land are of proposed lots, also the area remaining in the original parcel;
         C.   Existing and proposed streets, alleys and/or easements on or adjacent to the tract;
         D.   Available utilities (if public sewer and water source and sewage disposal system should be indicated);
         E.   Names of abutting property owners;
         F.   Proposed use of the land;
         G.   Certification of Registered Professional Engineer or Registered Surveyor as to the accuracy of survey and plat showing name, address, registration number and seal;
         H.   Approval Certificate. Place for the signature of the Chairman and Secretary of the Planning Commission, place for the seal of the Commission and the date of approval plus a place for the signature of the City Engineer and the date of his approval;
         I.   Draft of protective covenants whereby the subdivider proposed to regulate the land use in the subdivision and otherwise protect the proposed subdivision;
         J.   Statement by the owner dedicating streets and rights-of-way for public use; and
         K.   Certificate of approval of the storm sewer, sanitary sewer and sewage disposal system from the City Sanitary Board.
      (4)   The following additional data shall be submitted upon request by the Commission:
         A.   Subsurface and drainage conditions of the tract; and
         B.   Any other data pertinent to the plat.
   (b)   Preliminary and Final Plats. Conformance to the above requirements may replace the preliminary and final plat requirements.
   (c)   Approval and Disposition of Final Plats. Approval and disposition of final plats shall be in accordance with Section 1311.06
